Recruiting an RN Liaison & Charge Nurse for Inpatient Care and Hospital Referrals 

  • The Hospice Inpatient RN Charge Nurse & Hospital Liaison provides clinical leadership for daily inpatient operations while serving as a key link between hospice services and referral partners.

  • This role supervises inpatient nursing staff, manages on-call coverage, ensures compliance with clinical standards, and maintains accurate documentation, while supporting patients and families throughout the care experience.

  • In the liaison capacity, the Charge Nurse conducts clinical evaluations, determines hospice eligibility, and completes admissions in collaboration with hospitals and healthcare partners across Fresno, Madera, and Merced counties.

About the Position

The Hospice Inpatient RN Charge Nurse & Hospital Liaison is a hands-on clinical leader who keeps the inpatient unit running smoothly while serving as a trusted connection to hospital and community partners.

This role leads the day-to-day flow of care, supports and guides the nursing team, and ensures patients and families receive thoughtful, compassionate, and well-coordinated care.

The Charge Nurse manages staffing and on-call coverage, keeps documentation accurate and timely, and remains closely involved with patients and families—answering questions, offering guidance, and providing reassurance during critical moments.

Supervisory Responsibilities

Responsible for the direction, coordination, and performance management of assigned staff in accordance with Hinds LifeCare policies and applicable laws. This includes hiring and onboarding, work assignment and scheduling, performance evaluation, coaching, discipline, and resolution of employee concerns.

Referral & Admission Coordination

  • Respond to requests for clinical evaluations, consultations, and informational meetings from referral sources, patients, and families

  • Complete hospice admissions for both inpatient and outpatient services

  • Explain hospice philosophy, services, and levels of care, including General Inpatient (GIP)

  • Assess patient and family needs and goals to determine appropriate hospice placement

  • Coordinate with the interdisciplinary team and the Medical Director to support timely admission decisions

  • Complete informed consent documentation with patients and families

Hospital & Facility Relations

  • Support and maintain referral relationships with hospitals and facilities

  • Partner with the Director of Inpatient Hospice Services to support census goals

  • Track hospice patient hospitalizations and coordinate with case management and discharge planning teams

  • Follow up on referrals not admitted to hospice to provide additional education or clarification as needed

  • Attend hospital, facility, or family meetings related to patient care and admissions

Candidate Overview & Qualifications

Education and Credentials

  • Graduation from an accredited professional nursing program; BSN preferred

  • Current, active Registered Nurse (RN) license in the State of California

  • Minimum of three (3) years of clinical nursing experience in hospice, palliative care, acute care, or a related clinical setting

  • Three to five (3–5) years of progressive supervisory or charge nurse experience, preferably within an inpatient hospice or similar environment

Leadership and Clinical Competencies

  • Demonstrated ability to lead and supervise clinical staff, including staffing coordination and workflow management

  • Strong clinical judgment with the ability to function independently and make sound decisions related to patient care and staff oversight

  • Proven experience working collaboratively within an interdisciplinary care team

  • Effective verbal and written communication skills with staff, patients, families, and referral partners

Professional and Regulatory Expectations

  • Knowledge of hospice and home health regulations, including federal and state requirements, licensing standards, and industry best practices

  • Demonstrated sensitivity to cultural differences and commitment to equitable, patient-centered care

  • Ability to remain calm, professional, and supportive when working with staff and families in high-stress situations

  • Maintains strict confidentiality and demonstrates respect for patient privacy, belongings, and the care environment

Base Salary:

$125,000–$128,000 annually

Bonus:

Bonus: Up to 7% based on performance

24.5 days annually (FT, 40-hour workweek)
